It's for anyone starting a new device or treatment who wants hands-on guidance — newly diagnosed patients, caregivers, or those switching medications. No referral is required.
Bring the device or medication you'll be learning to use (for example your glucometer, inhaler or injection pen) so the nurse can guide you directly.

Follow-up care for individuals and families—chronic condition monitoring, pediatrics, results review, and prescription renewals when appropriate. In-person visits with telemedicine available for select concerns when clinically appropriate.
