  • Brief medical history focused on the main concern
  • Symptom assessment and clinical evaluation
  • Diagnosis and treatment plan when clinically appropriate
  • Prescriptions when clinically appropriate
  • Requisitions for tests (lab/other) when clinically appropriate
  • Clear next steps, red flags, and follow-up guidance
  • Write down your main symptom and when it started (timeline)
  • Bring a list of current medications and allergies
  • Have relevant info ready (temperature readings, home test results, photos of rash if telemedicine)
  • For urinary symptoms: note burning, frequency, fever, flank pain, pregnancy possibility (if applicable)
  • For sore throat/cold symptoms: note fever, cough, exposure, travel, and symptom severity
  • This visit is for one issue only; if more than one concern is addressed, or complexity is higher, it will be redirected to a Complex Medical Consultation
  • Telemedicine may be converted to an in-person visit if a physical exam is required
  • Not for emergencies: if you have severe symptoms (chest pain, trouble breathing, severe weakness/numbness, confusion, fainting, heavy bleeding), call 911 or go to the nearest emergency department
