Symptoms with uncertain diagnosis or requiring deeper assessment
Headaches or migraines (recurrent or severe patterns)
Anxiety or depression (assessment and management when appropriate)
Insomnia with significant impact
Non-urgent chest pain that needs careful evaluation
Abdominal pain requiring a more detailed approach
2–3 simple concerns in one visit, when clinically appropriate
More detailed medical history and review of relevant systems
Assessment of contributing factors (sleep, stress, medications, lifestyle) when relevant
Clinical evaluation and differential diagnosis discussion when appropriate
Treatment plan (medications, supportive care, referrals) when clinically appropriate
Requisitions for lab tests or imaging when clinically appropriate
Documentation or forms only if within the visit scope (otherwise may require a separate admin service)
Clear follow-up plan (what to monitor, when to recheck, red flags)
Bring/prepare a short timeline of symptoms (when it started, triggers, what improves/worsens)
List medications, supplements, allergies, and prior diagnoses
Bring relevant records if available (past labs, imaging reports, specialist notes)
For telemedicine: ensure good lighting, stable connection, and be ready to show visible symptoms (rash/swelling) if needed
Prepare 2–3 priorities max so the visit stays focused
Complex symptoms can still require in-person examination, testing, or referral
Telemedicine has limitations for physical exam (lungs, abdomen, neuro exam); an in-person visit may be recommended
Not for emergencies: if severe or rapidly worsening symptoms occur, call 911 or go to the nearest emergency department
Some conditions may require urgent escalation depending on findings (e.g., chest pain with concerning features)
