Choose this appointment if you are an adult seeking a physician-led medical assessment for possible ADHD. A Referral is not required for the ADHD consultation. The visit focuses on attention, organization, restlessness or impulsivity concerns and how they affect work, studies, relationships or daily responsibilities.
This is a medical clinical assessment. It is not a full neuropsychological evaluation and does not provide detailed cognitive testing. Further assessment may be recommended if the available information is not sufficient or if another condition needs to be considered. Any treatment or prescription decision is based on the physician’s assessment and is not guaranteed.
The consultation may include
Choose this consultation for persistent or recurrent fatigue, widespread pain, an existing diagnosis, or symptoms that may require a more detailed medical review. The physician considers how the symptoms developed, their effect on sleep and daily functioning, current medications, prior assessments and available results.
Fibromyalgia and chronic fatigue can involve complex symptoms and may not be confirmed or fully addressed in one appointment. Bloodwork, imaging, sleep testing, specialist care and other services are not included in the consultation fee and may need to be arranged separately. Telemedicine may be appropriate for some visits, but an in-person assessment may be required.
The consultation may include
Choose this consultation when a mental health concern is affecting mood, thinking, sleep, relationships, work or daily functioning and requires a longer medical assessment. The physician reviews the main concern, relevant history, medications, current supports and the effect of symptoms on daily life.
This is a physician-led medical consultation. It is not psychotherapy, a full psychological or neuropsychological assessment, or specialized psychiatric care. The physician may recommend follow-up, psychoeducation, psychotherapy, psychiatry, testing or another service when appropriate. Forms, reports and administrative requests are separate unless the clinic confirms otherwise. This service is not intended for urgent or crisis care.
Follow-up may include
Book this follow-up after an ADHD or mental health consultation with Dr Kourosh Lalavi, or when the physician has asked to review your progress. It is intended to assess changes since the previous visit and decide whether the current plan should continue or be adjusted.
This appointment is not designed for a new comprehensive assessment, several new concerns or crisis care. A new concern may require an initial ADHD, complex mental health, fibromyalgia and chronic fatigue, or general medical consultation. Prescription renewals and medication changes remain subject to clinical assessment. Some follow-ups may be completed by telemedicine across Québec. An in-person visit may be required when examination or testing is needed.

A confidential, structured clinical assessment with targeted questionnaires where relevant, a discussion of the probable diagnosis and a personalized treatment plan when indicated. It doesn't replace a full neuropsychological assessment or specialized psychiatric care.
Yes. ADHD and mental-health consultations are available by secure video anywhere in Québec, or in person in Brossard — whichever you prefer.
A follow-up is often recommended to review the plan and adjust as needed; it's booked separately at the follow-up rate (CA$245). Your physician will advise on timing.
Yes. You can book a private ADHD consultation directly — no referral required — in person in Brossard or online across Québec.
Yes. All consultations are strictly confidential and your information is protected under Québec's Law 25. You can also choose a private video setting from home.
