The physician reviews the circumstances of the event, your symptoms, medical history and relevant records. The physician performs the required examination and discusses treatment, functional or work limitations and the next steps. This appointment is appropriate for the follow-up of a CNESST or SAAQ claim that has already been assessed and is being managed by Bina Medical Clinic.

This medical examination is intended for individuals who have received a request from the SAAQ regarding the issuance, renewal or review of a driver’s licence.
The examination takes place in clinic. Bring the SAAQ letter and the exact form requested, including Form 28 when applicable. You should also bring your medication list, glasses or hearing aids and any relevant medical records.
One standard SAAQ form completed during the appointment is included. Laboratory testing, ECG, formal vision or functional assessments and specialist reports are not included.
